Ils se caract\u00e9risent par une perturbation du d\u00e9veloppement cognitif et\/ou affectif. Cr\u00e9\u00e9 en 2017\u00a0, le PCPE, \u00a0par les interventions des professionnels qui le composent, tentent de pr\u00e9venir \u00a0les ruptures de parcours, \u00e0 tout \u00e2ge, en organisant un accompagnement adapt\u00e9 aux besoins des personnes, quel que soit leur handicap.<strong><\/strong><\/span><\/p>\n<p><span style=\"color: #003366;\">Le P\u00f4le de Comp\u00e9tences, ouvert sur l&rsquo;Arrageois-Ternois, accompagne prioritairement une file active de 20 enfants, adolescents et jeunes adultes en situation de handicap, \u00e2g\u00e9s de 4 \u00e0 25 ans.<\/span><\/p>\n<p><span style=\"color: #003366;\">Ce dispositif coordonne la \u00a0mobilisation collective des acteurs du territoire \u00a0dans divers champs (sant\u00e9, sociaux et m\u00e9dico-sociaux, \u00e9ducation nationale, MDS, ASE\u2026) aussi bien en \u00e9tablissement qu\u2019en lib\u00e9ral.<\/span><\/p>\n<p><span style=\"color: #003366;\">Il s\u2019agit d\u2019un dispositif souple, adaptable et innovant qui apporte une r\u00e9ponse ajust\u00e9e aux besoins les plus complexes, en proposant aux personnes des projets personnalis\u00e9s d\u2019accompagnement dans une vis\u00e9e inclusive.<\/span><\/p>\n<p><span style=\"color: #003366;\">Le p\u00e9rim\u00e8tre d\u2019intervention est \u00e0 ce jour arr\u00eat\u00e9 au territoire de l\u2019Arrageois et du Ternois pour des \u00a0personnes en situation de handicap \u00e0 partir de 4 ans\u00a0:<\/span><br \/><span style=\"color: #003366;\">\u2013 N\u00e9cessitant un accompagnement sp\u00e9cifique dans leur parcours en milieu de vie ordinaire,<\/span><br \/><span style=\"color: #003366;\">\u2013 En situation de rupture (ou de risque de rupture) de parcours, en attente d\u2019une r\u00e9ponse d\u2019accompagnement effective et satisfaisante au sein d\u2019un dispositif adapt\u00e9,<\/span><br \/><span style=\"color: #003366;\">\u2013 Accueillis de fa\u00e7on non adapt\u00e9e dans le secteur sanitaire ou dans le secteur m\u00e9dico-social (situation complexe n\u00e9cessitant l\u2019intervention conjointe de plusieurs acteurs).<\/span><\/p>\n<p><span style=\"color: #003366;\">L\u2019accompagnement est transitoire et s\u2019entend sur une dur\u00e9e de 6 mois, renouvelable une fois et pour une dur\u00e9e de 3 mois si n\u00e9cessaire.<\/span><\/p>\n<p><span style=\"color: #003366;\">Pour \u00eatre accompagn\u00e9es, les personnes doivent b\u00e9n\u00e9ficier d\u2019une notification MDPH d\u2019orientation PCPE.
